SUMMARY
Family Service League is seeking a full-time Licensed Practical Nurse (LPN) for the Assertive Community Treatment (ACT) program, which provides behavioral health services for adults who have been diagnosed with a severe and persistent mental illness. The ACT program is comprised of a multi-disciplined team including skilled staff in psychiatry, nursing, and social work. The LPN will play an integral part of the team, working under the direction of the Registered Nurse the LPN will provide clients with medication management, health assessments and counseling. The Licensed Practical Nurse will be given an agency vehicle to use for ongoing home visits with the clients. Travel is required.

QUALIFICATIONS
Licensed Practical Nurse (LPN) with a current New York State License required.
At least 2 years of experience working with adults with serious mental illness and co-occurring substance use disorders required.
Excellent time management, interpersonal, and verbal and written communication skills required.
Proficient computer skills, including Microsoft Office required; experience with an EMR is preferred.
Valid and clean NYS Driver’s License required.
