Job Title: Medical Social Worker (Acute Care)

Location: Boston, Massachusetts

Job Type: Contract

Duration: 13 Weeks

Shift: 8:00 AM – 4:30 PM, Monday to Friday

Position Overview:

We are seeking an experienced and dedicated Medical Social Worker to join our client’s team, specializing in acute care settings. This role involves working with interdisciplinary teams to provide comprehensive psychosocial support to patients and families, addressing complex psychosocial needs, and supporting patients with chronic illnesses and conditions such as Sickle Cell Disease. The ideal candidate will have experience in medical social work, case management, and crisis intervention in an acute care environment.

Key Responsibilities :

• Psychosocial Assessment : Conduct comprehensive psychosocial assessments to identify patient and family needs, strengths, and resources in both inpatient and outpatient settings.
• Care Planning : Develop and implement individualized care plans, including psychosocial diagnoses and tailored interventions to support patients' medical and psychosocial goals.
• Counseling & Crisis Intervention : Provide counseling, emotional support, and crisis intervention for patients and families dealing with the acute and chronic impacts of medical conditions, including pain management and treatment adherence.
• Team Collaboration : Round with interdisciplinary teams to ensure integration of psychosocial support into patient care plans.
• Patient Advocacy & Education : Support, educate, and advocate for patients, promoting self-advocacy and resilience in managing chronic conditions like Sickle Cell Disease.
• Community Liaison : Serve as a liaison with external agencies to advocate for patients and connect them with necessary community resources.
• Mentorship & Program Development : Provide mentorship and coaching to patients, families, and team members, and participate in quality improvement initiatives and program development efforts.

Qualifications :

• Education : Master's in Social Work (MSW).
• Licensure : Licensed at the Masters Level.
• Experience : Minimum 2 years of post-MSW acute care experience in medical social work, case management, or related fields.
• Certifications : BLS (Basic Life Support) required.
• Preferred Experience : Housing experience, substance use disorder, grief and bereavement counseling, cultural responsiveness, crisis intervention.
