Hiring ASAP Care Coordinator/Social Workers

Job Type: Full-Time

Job Summary:

The Care Coordinator is responsible for assessing client needs, developing individualized service plans, providing short-term counseling, facilitating workshops, and connecting individuals and families with long-term resources. This role requires crisis intervention skills, collaboration with service providers, and support for individuals facing mental health challenges, substance abuse, homelessness, or other vulnerabilities.

Key Responsibilities:

• Conduct psychosocial assessments to evaluate the needs of individuals and families.
• Engage clients in short-term counseling, group sessions, and workshops.
• Identify and refer clients to appropriate community resources for long-term support.
• Provide crisis intervention services as needed.
• Identify individuals who require a higher level of service, including those experiencing domestic violence, mental illness, substance abuse, or high-risk behaviors.

Child & Family Support:

• Participate in classroom activities to observe children and assist in developing individual service plans.
• Engage parents in the service planning process and coordinate referrals for children to external services.
• Facilitate playgroups to address childhood trauma.
• Develop and facilitate workshops and support groups.
• Collaborate with teachers, recreation staff, and other service providers to ensure comprehensive support.
• Escort clients to appointments using public transportation when necessary.

Essential Qualifications & Requirements:

• Education & Licensure: LMSW/LMHC must be obtained within the first 30 days of employment and maintained

Salary 33.00 - 33.75/hr
